Guthrie is pleased to offer you member information via a Web-based version of our computer systems called eGuthrie.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms and Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

Response to Electronic Communication – Guthrie will make its best effort to provide a timely response to electronic inquiries. In some cases, the staffs that need to respond to an electronic inquiry or other communication may not be immediately available so a patient should allow 1-3 business days for a response. Accordingly, emergent or urgent situations requiring immediate attention should not be submitted electronically.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Minor Eligibility – An individual must be age 18 or older to request a personal account or age 13-17 with parental permission. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Minor children between the ages of 13 and 17 (inclusive) will be eligible to participate in eGuthrie services with parental/guardian permission. Also, a parent or legal guardian may have access to information regarding allergies and immunizations or will be granted full access with the child's permission.

Access Code – When you initially enroll in eGuthrie, you will be provided with an access code. The access code you receive will give you the opportunity to create a personal login name (called "eGuthrie ID") and a password. Once this access code has been used it will no longer be valid. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as it they were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that your security has been compromised. You may change your password at any time.

Proxy Access – Proxy access (access to another person eGuthrie) is granted according to Guthrie policy. The policy outlines the proxy requirements for 1.) Parent/guardian to child (under 13 and 13-17) and 2.) Adult to adult. Proxy policy and forms are available at your provider's office and

E-Mail Privacy – Patients who are users of eGuthrie should be aware that they will be notified via e-mail when there is new medical information to be viewed on eGuthrie. This means that any person with access to a patient's e-mail will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing eGuthrie may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.

Security and Confidentiality – We afford the same degree of confidentiality to medical information stored on eGuthrie as is given to medical information stored by Guthrie in any other medium. Guthrie is committed to protecting the confidentiality of your medical information. We limit Guthrie employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses eGuthrie. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

Appropriate Use – Access to the eGuthrie system is granted to users for the purpose of furthering their medical care. Use of the system unrelated to this purpose may result in the suspension or termination of access privileges. Improper activities include, but are not limited to, the use of inappropriate or abusive language, reserving appointment times that are not needed or are frequently cancelled and other activities that are deemed to cause a disruption to operations in the sole discretion of Guthrie.

As a Guthrie patient, you have the option to allow other health and wellness applications to access your personal health data in eGuthrie. These third party "apps" are useful to some patients' personal health goals.

If you would like to allow a third party to import your eGuthrie personal health data into their application, you must establish an eGuthrie patient portal account first. The username and password you set for eGuthrie will be used to authorize and authenticate any app you choose to allow to access your health information.

Guthrie reserves the right to disable any 3rd party application if needed.
